ANNUAL ENROLLMENT 2013:
October 1, 2012 to October 31, 2012 for a
January 1, 2013 effective date.

You may be eligible to join this plan if:

  1. You reside in any of the following parishes: (click to view).

  2. You are an eligible active State employee OR State retiree AND are not currently enrolled in one of OGB's Medicare Advantage Plans.

ANNUAL ENROLLMENT 2013:
October 1, 2012 to November 26, 2012 for a
January 1, 2013 effective date.

You may be eligible to join this plan if:

  1. You live in the State of Louisiana; and

  2. You are a retired State employee eligible to enroll in Original Medicare because of age or disability, currently enrolled in Original Medicare, or you are currently enrolled in an OGB-sponsored Medicare Advantage plan.
