2009 Enrollment has ended. 2010 Enrollment dates will be posted as soon as they are made available.
You may be eligible to join this plan if:

You wish to obtain ALL of your medical care from your choice of Medical Home Primary Care Physicians in Region 9.

You are an eligible active State employee OR State retiree AND are not currently enrolled in one of OGB's Medicare Avdantage Plans.
ANNUAL ENROLLMENT:
Begins November 15, 2009
You may be eligible to join this plan if:

You live in the State of Louisiana; and
-
You are a retired State employee eligible to enroll in Original Medicare because of age or disability, currently enrolled in Original Medicare, or you are currently enrolled in an OGB-sponsored Medicare Advantage plan.
